Spot Map

The spot map shows the path to our client*s residence from our point of reference which is the Liceo de Cagayan
University (LDCU) main campus (see legend). From the school we rode a Bulua route jeep liner. The fare was worth
P6.00 per head. As we made our way to our client*s residence, we passed by the following establishments: SSS, GSIS,
and LDCU high school campus an. As the jeepney approaches the Bulua, we then dropped off at overpass. From there,
we rode a single motor cycle on the way to macapagal road passing ABS-CBN going to our patient’s location which is at
the Soldiers Hill subdivision with a fare of P6.00 per head..
Scope and Limitations

The study was conducted throughout our psyche pre-exposure here at Cagayan de Oro city. The study involves
the care of a mentally ill individual residing in this local city. The client must meet the following criteria: he/she must have a
diagnosed psychosis and must have immediate family available as informants for the study.
The study was done for 5 days of home visitation. Consent was given prior to the conduction of the study.
Assessment was conducted and interventions were instituted according to the needs of the patient during the course of
nurse-patient interaction. The study only encompasses the health condition of the patient that was been assessed and
determined in the actual setting on that given span of time. The names used in the study are not the real name of our
client and informants to protect their identity and maintain confidentiality.
