Initial Setup for users on LCDX-ConNEX

#1) email@ of PRIMARY contact who should have complete access to patient files
#2) email@ to REQUEST FILES of video images be sent to LCDX Lab
email@ of anyone who would like to be CC on correspondence
#3) email@ to receive completed report from LCDX
email@ of anyone who would like to CC on correspondence

EXAMPLE of EMAIL Notifications:

Subject: Confirmation – Assigned Case# 08101.1.ABC

Thank you, a requisition has been received for patient: ABC. Please refer all
correspondence to case#08101.1.ABC for this patient.

Below is a link to send us only files for case#08101.1.ABC. In order for us to process a
Life Path-Predict Assay specimen, please do the following:
1) create file within LifeCell DX folder, name it 08101.1.ABC, create subfolder,
08101.1.ABC.Images and 08101.1.Report.
2) refer to LCDX Checklist: submit image files via this email link and “Semen
Collection Information Sheet” via fax number 716-408-330

Upon receiving all required information, you will receive an email to confirm your work
order has been processed and can expect a completed report within 48 hours. Any
