Dentinoma Is an extremely rare tumor of odontogenic origin composed of:
1. Immature connective tissue
2. Odontogenic epithelium 3. Irregular or dysplastic dentin. Clinical Features: Occurs predominantly Swelling is often present central in the mandible, for a variable period of especially in the molar time, pain, perforation of area, and frequently is mucosa and subsequent associated with an infection also have been impacted tooth. presenting complains. Radiographic Features: Findings are not specific , but usually there is a radiolucent area in the bone containing either a large, solitary, opaque mass or numerous smaller, irregular radiopaque masses of calcified material which vary considerably in size.
Findings are similar to those noted on the simple
odontoma as well as in ameloblastic fibro-odontoma. In some cases the dentin is present in relatively small quantities/poorly calcified so that there are no opacities in the radiolucency. Histologic Features: Composed of masses of irregular dentin which has been termed dentinoid or osteodentin. The connective tissue often resembles the dental papilla but the degree of cellularity varies.
It is difficult to justify the diagnosis of dentinoma
unless recognizable dentinal tubules are present, thus the occurrence of enamel or enamel matrix precludes the diagnosis of dentinoma. Treatment: Surgical excision is carried out w/thorough curettage of the area.
In some lesions they have a connective tissue capsule
which, if remnants are left at the time of operation , this may now be the basis of recurrence of the dentinma. Prognosis: Since the lesion is benign in that it never metastasizes the problem now would be that there may be considerable local destruction of bone by the tumor.
