    By signing this form I give permission to Student Activities & Leadership to check my records in order
    to confirm eligibility to serve as a member in the Cabinet. Furthermore, I agree to abide by the Student
    Leader Code of Conduct. I also understand that, if chosen for a position, I serve at the pleasure of the
    President of the Undergraduate Student Government and must continue to meet the required criteria set
    in the governing documents of the USG and the Student Leader Code of Conduct throughout my
    tenure. Lastly, I understand this is a volunteer position and that if I wish to vacate office, it is requested
    I do so with at least 2 weeks notice given in writing to the President of the Undergraduate Student
    Government.
