Request for the issuance of special patent Re Special Law/Act of Congress Comments and recommendation of the following: DEPARTMENT OF HEALTH Provincial Health Officer recommended by the Regional Health Director DEPARTMENT OF PUBLIC WORKS AND HIGHWAY District Engineer Survey of the land to be reserved duly approved by the Regional Technical Director for Lands and approved plan. Verified and certified correct technical descriptions. Certified photographs (visual documentations) showing panoramic view of the area subject of the proposed special patent as well as adjacent areas, vicinities, improvements introduced, land marks, identifying features, settlements, etc. Land classifications or municipal index maps projecting the area covered by the proposed special patent and land uses of the area and surrounding vicinities within two kilometers radius; Narrative report of investigation with recommendation officially endorsed to the Secretary by the Regional Executive Director through the Undersecretary for Field operations
