Dear Mr. ______________:

We are pleased to inform you that we are willing to render our professional and
consultancy services for the application for registration of your company (owning a tract of land
in Mauban, Quezon) with the Tourism Infrastructure and Enterprise Zone Authority
(TIEZA) as a Tourism Enterprise Zone.

As such please find below the Scope of Works for the above-mentioned services:

A) Preparation of the following requirements:

a) Duly accomplished TIEZA Application Form;


b.) SEC Registration Certificate, copies of the articles of incorporation and by-laws; in
case of a joint venture, a copy of the joint venture agreement;
c.) Company Profile, containing basic data/ information on its technical, financial,
marketing and management capability/ competence to undertake the proposed project;
d.) Resolution of the applicant's board of directors authorizing the filing of the
application and the designation of their authorized representative to TIEZA; list of its directors,
principal officers, and major stockholders, including their respective bio-data.
e.) Vicinity Map reflecting various land uses, important verifiable landmarks within the
five (5) kilometers radius of the project site, proximity to transportation and utilities
infrastructure, and tourism focal points in the vicinity and their distances from the project site;
f.) Proof of land ownership and/ or Long-Term Lease Agreement(s) for a period of not
less than twenty five (25) years on the entire area of the proposed TEZ;
g.) Endorsement letter from the National Historical Institute (NHI) in the
case of Cultural and Heritage Tourism Zone, from the Department of Health (DOH) in the case
of Health and Wellness Tourism Zone; or from Philippine Retirement Authority (PRA) in the
case of retirement villages/communities; and

h.) Other supporting documents/ papers/ clearances as may be required by the TIEZA
depending upon the nature of the business and the type of business organization of the applicant.
i.) Resolution of the concerned LGU approving the development plan;
j.) Copy of the approved Comprehensive Land Use Plan of the concerned LGU;
k.) Verified survey returns and technical description of the land area for the proposed
TEZ;
l.) Environmental Compliance Certificate issued by the Department of Environment and
Natural Resources (DENR);
m.) Clearance/Permit from the concerned Protected Area Management Board if the
proposed zone is within a Protected Area;
n.)Financial statement for (3) years for existing companies;
o.) Development Plan of the area which shall identify and include the following:
1. Tourism focal points and resources available within the proposed TEZ and adjoining
areas;
2. Features which satisfy the requisites for the designation of a TEZ enumerated under
Section 2, Rule II of the Rules;
3. Amount to be invested by the applicant in the area;
4. Areas for infrastructure development and the kind of development, for investment and
the nature of investment, and for preservation and the sustainable activities allowed within
preserved areas;
5. Public utilities to be operated within the TEZ;
6. Medium and long-term studies on market trends, and corresponding development
strategies for the TEZ;
7. Studies on the economic impact of development within the TEZ and in surrounding
communities;
8. Studies on the carrying capacity of the TEZ and surrounding communities;
9. Design plans for structures incorporating therein measures that should ensure the
sustainable development of the area and the surrounding environment, to include the following:
the civil and architectural plans that preferably incorporates Philippine design elements and
cultural heritage, structural plans, electrical, mechanical, sanitation and design computations,
cost estimates and specifications, financial schedules and construction timetable;
10. Energy efficiency and climate change mitigation measures to be adopted and
implemented in the area; and
11. Compliance statement on Development Standards.

p.) Other requirements as may be prescribed by the TIEZA Board.

B) Filing of the application and submission of the documentary requirements;


C) Publication of the application;
D) Meetings and representation to the Local Government Officials and the TIEZA
officials and staff;
E) Follow up of the application up to the TIEZA Board;
F) Pick up/release of the Certificate of Designation

As a TIEZA-registered Tourism Enterprise Operator, you will be entitled to several


incentives. Please see attached for the incentives given by TIEZA.

For the services rendered in the application for registration with TIEZA as a Tourism
Enterprise Zone, we will be charging a fee amounting to Pesos: One Million Only.

The consultancy fee shall be paid as follows:


1) Upon (table) survey of the property by a Licensed Geodetic Engineer, and issuance of
the verified survey returns and technical descriptions of the land area - PhP 100,000;
(Same with the Agro-Industrial Ecozone, a Consolidation Survey duly approved by
the DENR Surveys Division is also a requirement, Sir.)

2) Upon issuance of the Environmental Compliance Certificate (ECC) – PhP100,000;

3) Upon completion of the vicinity map reflecting various land uses, important verifiable
landmarks within the five (5) kilometers radius of the project site, proximity to
transportation and utilities infrastructure, and tourism focal points in the vicinity and
their distances from the project site – PhP100,000.00;

4) Upon completion of the design plans for structures incorporating therein measures
that should ensure the sustainable development of the area and the surrounding
environment, to include the following: the civil and architectural plans that preferably
incorporates Philippine design elements and cultural heritage, structural plans,
electrical, mechanical, sanitation and design computations, cost estimates and
specifications, financial schedules and construction timetable – PhP100,000.00

5) Upon completion of the development plan – PhP50,000.00

6) Upon filing of the application and submission of the documentary requirements –


PhP50,000.00

7) Upon release of the Certificate of Designation – PhP500,000.00

Once you approved of this Sir, we will request a down payment/mobilization fee
amounting to 10% of the Contract Price (PhP100,000.00) to pay the Engineer for the
preliminary works to be done such as the consolidation survey, the preparation of and
submission of documents for the ECC and preparation of the design plans (civil, architectural,
structural, cost estimates, etc). The said down payment shall be deducted from the final payment
upon release of the Certificate of Designation.
