    bonded wire by using cutting pliers. The 1.6 mm galvanized wire snapped and
    struck the P in his right eye which was not protected due to improper wearing of
    his safety glasses.
    Corrective Action
    SAFETY GLASSES MUST BE WORN PROPERLY ALL THE TIME
    WHILE ON SITE. YOU ONLY HAVE ONE SET OF EYES
