    Greetings!

    I, Bettina Camille B. Laranang of BSN IV, Block J, Group 4, am here to explain the reason why I was not able to submit the Competency Checklists and Templates. I was not able to complete the signatures needed on one of my immediate newborn cases which I had when I was in my second year. Back then, I was able to accomplish the needed information on the newborn care slip as well as the signatures, however, I was not able to accomplish 2 copies and there were also mistaken entries so I had to redo it again. I was not able to get the signatures of the CI and Staff nurse concerned because of conflicts in schedule. Another is the Competency Checklist for Immediate Newborn Care wherein the first column was not yet accomplished. Hence, this letter of explanation.
