FOR THOSE UNDER 18 YEARS OF AGE For consideration of my performance in the educational Program entitled ___________________________ (the Program), and without any further consideration from Ball State University (Ball State), I hereby grant permission to Ball State to utilize my performance in connection with the Program for any and all educational or instructional purposes of Ball State University throughout the world in perpetuity. I agree that my participation in the Program may be edited in Ball States sole discretion. I consent to use of my name and that of my band and biographical material about me in connection with Program publicity and related institutional promotional purposes. I expressly release Ball State, its agents, employees, licensees and assigns from and against any and all claims which I have or may have for invasion of privacy, defamation or any other cause of action arising out of production, distribution, broadcast or exhibition of the Program.
