Media Liability Release

For good and valuable consideration, the receipt of which is acknowledged, I hereby
expressly grant to JYP Entertainment and to JYP Entertainment’s licensees, successors, and
assigns the right throughout the universe in perpetuity to film and photograph the participant,
the participant's voice, conversation and sounds, and/or otherwise reproduce the participant's
physical likeness in, and in connection with the exhibition of , any visual and/or audio-visual
work featuring or promoting or derivative works thereof and for advertising, promotion, or any
other lawful purpose whatsoever. I understand and agree that you shall be the exclusive owner
of all copyright and other rights in and to such photograph, film, tape or other recording of the
participant's voice or physical likeness, and you and your licensees, successors and assigns
shall have the unlimited right to copyright, publish, exhibit and otherwise exploit the
production or any part thereof in any manner you desire, in any manner or media now known
or hereafter developed. I further grant you and your licensees, successors, and assigns, the right
to record and exploit in any manner whatsoever throughout the universe in perpetuity any
recordings made by you of the participant's voice and all instrumental, musical, or other sound
effects produced by the participant as the same may be embodied in such production. I hereby
waive any rights of privacy or publicity, whether granted by statute or common law, that the
participant may have in connection with the production. I understand and agree that the
participant will have no right or review or approval in connection with your use of the
production or any derivative works thereof and that the rights granted herein include the right
to alter, edit or distort any or all elements of the production in any manner. I warrant and
represent that I have the right and power to enter into this agreement, and grant the rights set
forth herein. I have obtained my parent (or guardian)’s consent to participate in this audition.
I hereby release and waive all claims and responsibility associated with this activity, which I
may have with JYP Entertainment, its employees, manager, and supervisors.

I hereby certify and represent that I have read the forgoing and fully understand the meaning
and effect thereof and, intending to be legally bound, I have hereunto set my hand on this day of

JYP Entertainment and KPOPCENTER (hereinafter referred to as “the Company”) cherishes

your personal information and complies with the law regarding “information communication
usage promotion and information protection law.” The Company informs you how we will treat
the personal information you provide and which measures will be carried out for personal
information protection through personal policy. The Company will post a notice on official
website (or individually) in case of amendment of privacy policy.

Article 1. (Collecting Personal Information)

Article 2. (Purpose of Using Collected Personal Information)
The Company collects the personal information to verify the identity of the user and to evaluate
the applicants for the purposes of audition.
Article 3. (Period of Storage and Using Personal Information)
As a rule, the Company promptly destroys personal information when collected and the
purpose of use is complete. However, the following information can be stored for a specified
period of time for the following reasons:

- Prevention of misuse
Electronic and hard copies of Application Form: Store for one (1) year
2) Relevant Law
The Company will store personal information in case of necessity prescribed by the related
laws and regulations such as commercial law and electronic commercial law for a certain
amount of time. In this case, the Company will use the information only for purpose of storage
and the period of storage is described below.
- Electronic commercial law
Records on customer complaints or dispute settlements: Store for three (3) years
- Information communications network act
Records on identification: Store for six (6) months
- Protection of communications secrets act
Records on visiting: Store for three (3) months

(California), involves a certain degree of risks that could result in injury, loss, or damage to a
person or property to the participating minor (the “Minor”). After taking into consideration the
risk being involved, on behalf of the Minor as his/her parent or legal guardian, I consent to the
Minor’s participation in this audition and/or entry into restricted areas and hereby accept and
assume all such risks, known and unknown, and assume all responsibility for the losses, costs
and/or damages following such injury, disability, paralysis or death, even if caused, in whole or
in part, by the negligence of the minor named above.

In addition, I hereby release and waive all claims and responsibility associated with this activity,
which the Minor and/or I may have with JYP Entertainment, KPOPCENTER (only for California
global audition participants), their respective employees, manager, and supervisors (the
“Releasees”). I agree that JYP Entertainment can use pictures or videos of participants in its
publicity efforts. If, despite the release, I, the Minor, or anyone on the Minor’s behalf, makes a
claim against any of the Releases, I AGREE TO DEFEND, INDEMNIFY AND SAVE AND HOLD
HARMLESS THE RELEASES and each of them from ANY LITIGATION EXPENSES, ATTORNEY
FEES, LOSS, LIABILITY, DAMAGE, OR COST THEY MAY INCUR DUE TO THE CLAIM MADE
AGAINST ANY OF THE RELEASES, WHETHER THE CLAIM IS BASED ON THE NEGLIGENCE OF
THE RELEASES OR OTHERWISE.

I HAVE READ THE ABOVE MEDIA LIABILITY RELEASE AND PRIVACY POLICY, AND
UNDERSTAND THAT BY SIGNING THIS WAIVER OF LIABILITY, I GIVE UP SUBSTANTIAL
RIGHTS I AND/OR THE MINOR WOULD OTHERWISE HAVE TO RECOVER DAMAGES FOR
LOSSES OCCASIONED BY THE RELEASEES’ FAULT, AND SIGN IT VOLUNTARILY AND WITHOUT
INDUCEMENT.
