What would be the benefits, I will get if I get registered with Truworth Health? You will get the confirmation of your empanelment with a Unique Identification Number. 6. What would be my employment status with the company? You would be designated as Associate Consultant and would be on part time employment with the company. 7. What would be my basic work / responsibilities by getting associated with Truworth Health? At the moment, we are foreseeing some or all of the work assignments mentioned below: - Corporate Consultation (part time) visit to corporate house/s on part time basis for providing consultation - Corporate Consultation (full time) visit to the corporate house/s on part time basis for providing consultation - Corporate Health Talks visit to corporate to deliver health talks / lectures - Tele Consultation telephonic consultation during the time of the day, given by you to staff of our corporate clients - Web Chat answering queries / consultations through web chat at the comfortable time of the day, as chosen by you, for staff of our corporate clients - Home Visits - Biometric Screening Report Preparation The software generated report to be prepared after assessing the findings of different basic health parameters, with comments and analysis - Quality check of Biometric Screening Reports To monitor the quality of the reports generated by critically going through them There may be role enhancement and other work options in due course of time. 8. Will I be provided with the Standard Operating Procedures (SOPs) to fulfill the tasks? The SOPs are being designed for all the processes and would be shared with you shortly, once you get empanelled with us.

9. Will I be provided the training at initiation of the assignment? Yes, you would be provided online training before you initiate the assignments. 10. Will there be any constraint of the minimum time to be devoted every day? There is no constraint of the minimum time to be devoted every day. It is flexible and as per your available slots. 11. Will I have to work as per some fixed schedule? The schedule would be fixed if you have to make visits to the corporate for consultation. Rest of the assignments would have the flexibility of timings. However, timings are to be honored for an assignment like health talks, home visits etc. 12. Can I work from the location of my preference? Yes, you can work from any location of your preference for online services, be it your residence, clinic, hospital or any other location with internet connectivity. 13. How will I get to know when my services are required? - There will be a defined schedule for onsite corporate visits. - For health talks and related sessions you would be notified at least 7 days in advance. - For online consultations / web chat, you would be provided with a user name and password and you may make yourself available as per the timings mutually decided. - For report preparation of biometric screening, you can login with your details as and when you get the time and carry out the work. - For home visits, you would be made aware on case to case basis as and when required. 14. Are the health talks carried out online? Health talks are generally carried out at the site i.e.; premises of the corporate clients for a group of people. However, there may be possibilities of addressing / interacting with a group of people through web conference / Google + etc in future. 15. How will I have to consult the patients? One to one consultation with the patient would happen during the visits to corporate. The option of providing consultancy / answering queries is also available through web chat or over the phone.

16. What is generally the nature of consultation, I have to offer to corporate clients of Truworth? Corporate clients seek the presence of Medical Doctor in their premises so that staff can seek advice on common ailments like fever, cold, cough, head ache, stomach ache, acidity, get BP measured etc. The consultants role is to guide and counsel staff members on chronic and lifestyle diseases, as well. 17. Can I continue my association even if I shift to any other location / city? Yes. You may continue to remain associated with us. Geographical location is not a constraint at all for online / telephonic assignments. For other assignments, update your records with us, so we may allocate whenever there is any assignment in the new city of your residence. 18. Can I have any full time opportunity to work with Truworth Health? Yes, it would be a pleasure. Depending on the requirements, which arise from time to time, we will get in touch with empanelled doctors for associating with Truworth Health on full time basis. 19. My speciality is not mentioned in the online empanelment form, can I still be registered? Yes, you may get empanelled and mention your speciality in the space given for other specialties. 20. My city is not mentioned in the list of cities, in the online empanelment form, am I not eligible to get registered? You are very much eligible. Select the state where you live and mention name of your city in the space given for other cities. 21. Can I get associated with you if I do not have a valid registration? The registration no. of a professional body is mandatory. 22. Can I get associated if I am presently studying in the professional course but not have the registration? The registration no. of a professional body is mandatory. 23. Would my personal information like address, photograph, contact no., email id, age and gender be displayed on the website? We would be taking consent from you in writing before publishing your personal information on the website. Only the personal information, which you would like to be posted, will be displayed on the website. 24. Why is it compulsory to provide photo in Blue, Grey, White background? The professional fee would be competent as per the market norms. The fee schedule would be fixed as per the type of assignments you opt to undertake. The detailed fee schedule would be shared following your empanelment with us. 26. What would be the frequency of the payment? The payments would be made on monthly basis. The payment would be made by 10th day of the following month for the services rendered in the previous month. 27. How would the payment be made? The monthly payment would be made through the funds transfer in your bank a/c. You need to share the bank a/c no. with the other details like name of the bank, branch and its IFSC Code.
