Diesel Tank survey work

1. Permit to work system - Confined space is identified as a tank previously contains diesel and height is 20 meters. 2. A proper evaluation of risks and hazards need to be done on site. 3. All personnel who involved the work must have experience and training (entrant, attendant, supervisor, confined space assessor, rescue personnel. Confined Space Assessor to certify fit for entry. 4. Remaining residual oil must be drain out. 5. Gas check must be performed before enter the tank. - Oxygen deficiency and enrichment. - According to diesel SDS, diesel vapour, naphthalene, xylene, nonane, benzene, hydrogen sulphide could be presence. - Carbon monoxide could be presence (near generator, drive way, exhaust outlet, etc.) 6. 4 in 1 multi gas detector and Toxic gas detector tube have to be used with extension hose, probe and pump. 7. If Gas meter extension hose or probe cannot be reached sample area, wear full breathing apparatus and enter into awkward places to gas check. 8. Always test the air at various level to be sure that the entire space is safe. 9. Oxygen (O2) 19.5% to 23.5% CO <25 ppm Flammable gas/ vapour <10% LEL (non-toxic) <1% LEL (toxic) H2S long term <10 ppm, short term <15 ppm If Gas check level is over above the range, purge the air by using effective and suitable forced ventilation. 10. After ventilated the air, gas check retest has to carry on. 11. Verify that electrical equipment used are 55V (AC) or 110V (DC). 12. Entry Permit has summited to authorized Manager by supervisor. 13. Need to display the entry permit at entrance. 14. Supervisor has to conduct tool-box meeting and brief all the workers. 15. Suitable and sufficient lighting must be used. 16. Air supplied respirator ( airline) and safety line ( life line ) must be used. 17. Need to monitor the air quality with personal gas detector continuously during work inside tank. 18. If isolation is required, LOTO ( lock out Tag out ) must be established. 19. Close pipe by installing a solid cap or plate (blanking & blinding) (inlet and outlet pipe may be there at the tank.) 20. Effective guard-rails or barriers have to be installed near access entry to the tank.

21. Personal Protective Equipment must be used. - Safety glasses, Safety shoes, helmet, safety harness, Chemical resistance clothing and glove, etc. 22. Communication devices must be used ( such as walkie talkie ) 23. Maintain adequate ventilation at all times. 24. After leaving the tank, ensure all personnel left the place.
