Dear Mr. Marcus:

This is in response to DOT Document Request No. 4 dated March 22, 2004, in which you
request:

1. All records, lists, and other documents produced under the direction of Dave
Canoles in the Air Traffic Managers office on the 10th floor of the FAA
Headquarters Building in Washington, D.C. on September 11, 2001, as referenced
on page 21 of Pamela Freni's book Ground Stop: "Mr. Canoles reminded
everyone that whatever was said, any instructions given, must be remembered and
recorded... .Later an exact listing of all questions and answers were meticulously
committed to a laptop computer."

On September 11, 2001, a teleconference was established in the conference room


adjacent to the Office of the Director of Air Traffic, located on the 10th floor of the FAA
Headquarters Building. Mr. Canoles ran the teleconference.

Initially, information received and relayed was captured on a board in the conference
room. At some point, due to the volume of material on the board, a laptop computer was
set up to capture information. The information captured became the "Chronology of
Events September 11, 2001".

I confirmed with the author of Ground Stop, Pamela Freni, that the reference she made in
her book was actually based on information provided by Mr. Tony Mello, who worked
for Mr. Canoles on September 11,2001. Mr. Canoles and Mr. Mello confirmed that what
was described to Ms. Freni was the information that became the "Chronology of Events
September 11, 2001". This document was provided to the Commission on at least two
separate occasions - most recently on November 17, 2003.

I queried other personnel involved in the teleconference for any additional notes, records
or other documentation and located handwritten notes taken by Air Traffic staff
personnel. In addition, I located a document that appears to be an early iteration of the
"Chronology of Events" but the document is titled "11 Sep 01 Catastrophic Crisis" and is
not dated. The documentation is enclosed. To the best of my knowledge, the enclosed
information, along with the "Chronology of Events" previously provided, represents the
documentation that exists from the teleconference.
