Childrens Services Program Inserts i-WRAP (Assertive Outreach Team) Our team provides services to young clients in order

to help keep them in their own homes and communities. This is accomplished through services that are individualized to each patient, and youth and family oriented. Our priority is reducing risk factors and fostering resiliency within each patient so that they might be able to better adapt to future situations. By working as a team, we are better able to focus on the child/whole family and be positioned to address issues that are negatively impacting their wellbeing. Each team is comprised of a Prescriber, a Licensed Mental Health Technician, a Social Service Worker, and Case Managers. The team will diligently collaborate to deliver the treatment and support that is required by the clients, and in order to aid in skill development. I-WRAP Team clients have access to services 24 hours a day 365 days a year. Case Management At Valley Mental Health we provide differing levels of case management according to the clinical needs of each patient. It is offered to assist the family in connecting with services and agencies within the community that can provide additional supports to their child and family. Each client, both adult and child, is assigned a care coordinator. This coordinator provides targeted case management for patients with severe and persistent mental illness and/or severe emotional disability. (SED?) Service will be provided in a flexible continuum reaching between outpatient services to in-home skills training programs. In-Home Services Behavorial support services are offered by Social Service Workers in the home of the child in order to implement supports in the home to maintain the child in their home environment. Each childs parents are responsible and in charge of their families and should be provided services that support, encourage, and assist them in their parenting role. Our desire is to simply aid and encourage the parents to further the ability of the child to remain in their home environment. Individual and family therapy can also be provided in the clients home to provide additional support to both client and family. Transition Services The main goal of our Transition Services program is to establish an environment of CALM (Coordinate, Access, Link, and Monitor) for its clients. A select number of our key services will help you to transition back to traditional living at any stage of life. Case management services involve addressing school and medical issues. In-Home services are driven by the patients therapeutic goals and needs. Lastly, our project reconnect works with independent living-aged youth. Outpatient Outpatient treatment focuses on decreasing a childs psychiatric symptoms in order to enhance and maintain the childs level of functioning.
