theeventofdeath.Therationaleis,ifyoudidnotmakeanynominationforyourEPFsavings,yournextofkinwillfacedifficultiestoclaim
yoursavingsfromtheEPF.

WhoCanBeNamedAsANominee?
Youcannominateanynaturalperson'asanominee/administrator.Anynaturalperson'referstoindividualsanddoesnotreferto
organisations,associations,societies,welfarebodiesandothers.
Youareadvisedtonominateyournextofkinsuchasyourspouseorchildrenandparentstoreceive/administeryoursavings.The
nominationcanstillbeacceptedifyounamedotherthanyournextofkin,howevertheremaybedifficultiesinthefuture,suchas
claimsfromyournextofkin.

NominationForMuslimMembers
WhenaMuslimmemberoftheFunddies,theperson(s)nominatedshallreceivethecreditofthedeceasedmemberasadministrator
(Wasi)whowillberesponsibletodistributethecreditinaccordancewiththeFaraidLaw.

NominationForNonMuslimMembers
Thenomineeistheactualrightfulbeneficiarywhowillreceivethecreditsofthedeceasedmember.
FornonMuslimmembers,intheeventofadeceasednominee
Beforethedeathofthemember,theeligibleapplicantisthenextofkinofthedeceasedmember.
Afterthedeathofthemember,theeligibleapplicantisthenominee'snextofkin.Thisisbecausethenomineeistheactual
rightfulbeneficiaryofthedeceasedmember'ssavings.

Youarerequiredtosubmitthefollowingdocumentswhichmustbecompleteandinorder:

AcopyofIdentificationCardeither
AcopyofMyKad/PoliceIdentificationCard/MilitaryIdentificationCard/PermanentResidentIdentificationCard(MyPR)
OR
AcopyofPassport(ifyouarenotaMalaysiancitizenwhohasbecomeanEPFmemberbefore1August1998)
ThenominationapplicationformdulycompletedhastobesubmittedbythememberatanyEPFOffice.
NOTE:
TheNominationForm(KWSP4)mustbereceivedbytheEPFwhenthememberisstillalive.

ELIGIBLEWITNESSES
Youcanchooseanyoneaged18yearsandabovetobeawitnesstoyournominationEXCEPTthepersonnamedinthenominationform,
EPFcounterstaffandEPFstaff.

HowCanYouRevokeYourNomination?
Thenominationmadeshallberevoked:
Uponthedeathofthenominee(s)duringthelifetimeofthemember.
ByaNoticeofRevocationofNomination(KWSP4A)
MembersubmitssubsequentNominationForm(KWSP4)
NOTE:
Thenominationshallalsoberevokeduponthemember'sfullwithdrawalunderLeavingtheCountryWithdrawal,Incapacitation
WithdrawalandPensionableEmployeesWithdrawal.
ANominationshallnotberevokedbyanyWill.

HowDoIUpdateTheNomination?
Thenominationcanbeupdatedfromtimetotimebeforeyoureachtheageof75.Updatingofthenominationcanbemadebysubmittinga
newNominationForm(KWSP4).ThenewKWSP4Formwillrevokeyourpreviousnomination.

HowCanICheckMyNomination?
Youcancheckyournominationviathefollowing:
