Abstract
Parkinson disease (PD) is usually associated with neuropsychiatric complications such as
depression, anxiety and apathy. Besides, psychiatryc symptoms are one of the most common side
effects of antiparkinsonian drug-therapy. Depression is the most frequent emotional disorder
reported in patients with Parkinsons disease (PD). Up to 20% of parkinsonian patients meet DSMIV criteria for major depressive episode and another 20% for dysthymia, while the prevalence of
depression in normal aged population is about 2-8%. The relationship between PD and depression
has not been fully established. Some investigators have suggested that depressive symptoms in PD
are causally related to the underlying neuropathological process, affecting predominantly
serotoninergic and dopaminergic pathways. Alternatively, depression in PD may represent a normal
reaction to the progressive physical impairment induced by the disease. Otherwise, up to 20% of
parkinsonian patients present levodopa-induced psychiatric complications. Visual hallucinations are
the commonest, but delusions, confusional states, sexual disorders and sleep disorders have also
been described. Serotonine and dopamine have been implicated in the neuropathological basis of
these disorders. To sum up, this paper reviews the main neuropsychiatric disorders associated with
PD and describe the neuropathological hypothesis proposed to explain these symptoms.
Resumen
Con frecuencia la EP se complica con una amplia variedad de trastornos neuropsiquitricos que
incluyen, entre otros, la depresin, la ansiedad y la apata. Adems los pacientes con EP pueden
presentar complicaciones psiquitricas como efecto secundario de los frmacos utilizados en el
tratamiento de la enfermedad. La depresin es el sntoma psicopatolgico no cognitivo ms
frecuente de cuantos aparecen en la EP alcanzando a prcticamente el 50% de los pacientes con
EP. La aparicin de sntomas depresivos parece relacionarse con factores exgenos como la
progresiva incapacidad motora que limita las actividades cotidianas de los pacientes y con factores
endgenos que probablemente incluyen alteraciones en los sistemas de neurotransmisin
dopaminrgico, noradrenrgico y serotonrgico. Aproximadamente el 20% de los pacientes con EP
presentan complicaciones psiquitricas inducidas por la levodopa. Las alucinaciones visuales son

Con frecuencia la EP se complica con una amplia variedad de trastornos
neuropsiquitricos que incluyen, entre otros, la depresin, la ansiedad y
la apata. Adems los pacientes con EP pueden presentar
complicaciones psiquitricas como efecto secundario de los frmacos
utilizados en el tratamiento de la enfermedad. Las ms frecuentes son,
alucinaciones, delusiones, trastornos del estado de nimo, ansiedad,
cuadros confusionales y alteraciones de la conducta sexual (CastroGarcia, 1997).
2. TRASTORNOS NEUROPSIQUITRICOS ASOCIADOS A LA EP
2.1. Depresin
La depresin es el sntoma psicopatolgico no cognitivo ms frecuente
de cuantos aparecen en la EP. Aunque la prevalencia de la depresin en
la EP varia entre un 12% (Rondot et al. 1984) y un 90% segn estudios,
de manera global se aceptan los datos de Gotham y cols (Gotham et al.
1986), y Cumming y cols (Cummings, 1992), quienes en una revisin de
catorce estudios, encontraron que prcticamente el 50% de los pacientes
con EP presentan sntomas depresivos, cumpliendo la mitad de los casos
criterios de episodio depresivo mayor segn la clasificacin DSM-IV.
El perfil clnico de la depresin en la EP incluye sntomas tpicos como la
perdida de inters por las relaciones sociales, familiares e
interpersonales, junto a otros ms atpicos, como la coexistencia de un
grado elevado de crisis de pnico y trastornos por ansiedad asociados en
ocasiones a un sentimiento de infravaloracin y cuadros psicticos
(Schiffer et al. 1988). Cummings y cols (Cummings, 1998a), en un
estudio de 40 pacientes con EP encontraron que el 90% de los pacientes
