& PwD 3 stands for Locomotor Disability & Cerebral Palsy.

d. Please read carefully the information bulletin before feeling up the form.
e. The bulletin of information is available in accessible formats on the website of University of Delhi
www.du.ac.in and Equal Opportunity Cell eoc.du.ac.in which could be read through Screen Readers and
in Braille & Audio format at the respective admission information centers for consultation purpose only.
f. Candidates are requested to select their course strictly according to their interest and willingness.
g. Every candidate is required to have the following documents ready for filling up of the form:

Class X Board Examination Certificate
(ii)
Class X Mark- Sheet
(iii)
Class XII Marks Sheet
(iv)
Class XII Provisional Certificate/ Original Certificate
(v)
Character Certificate (recent)
(vi)
SC/ ST Certificate ( in the name of the candidate) issued by the competent authority. (if applicable)
(vii) Disability certificate issued by the competent authority.
(viii) OBC (Non- Creamy Layer) Certificate ( in the name of the Candidate) issued by competent authority.
(if applicable)
(ix)
Transfer Certificate from School/ college as well as Migration Certificate from Board/ University are
required from those students who have passed senior secondary exam from outside Delhi.

At least two passport size self attested photographs.
