Ruiz Street, San Juan Metro Manila Dear Mrs. Mejia: I have to address myself to your goodself because yesterday, January 7, 2005, Spouses ELEAZAR C. SALLOMAN and ELIZABETH L. PUTAN came to me and voiced their predicament for submitting to your good office a copy of their Marriage Contract where it appears an imposition of the month of October to the month of September as their wedding month. From a quiet inquiry and verification made by the undersigned, it became apparent to me that actually the undersigned solemnized the wedding of the said couple last September 4, 2004, but, for one reason or another the papers to show this date of solemnization was misplaced or lost by our liaison/representative, one GLORILIE TUMAMPIL before it was reported to the Office of the Civil Registrar of Quezon City, thus, the necessity of producing another set of documents signed again by the parties involved, which in effect caused so much delay in reporting the Marriage Contract. Our own record shows that couple ELEAZAR C. SALLOMAN and ELIZABETH L. PUTAN accomplished their application for Marriage License on August 23, 2004, thus, the regularity of solemnizing the marriage last September 4, 2004 here in our office (located at 96-D G. Araneta Avenue, Quezon City). Enclosed is an Affidavit of Late Registration re Certificate of Marriage of the above-named spouses executed by our liaison officer, GLORILIE TUMAMPIL, admitting her miscarriage and apparent loss of this important papers of the wedding of ELEAZAR C. SALLOMAN and ELIZABETH L. PUTAN last September. It is regrettable that the truth of this matter was kept from me by our liaison representative perhaps to avoid scolding or disciplinary measures but which is in effect, to put things in order, there was reconstruction of documents that ultimately affected the actual date of ceremony, thus, this necessity of our liaison officer to execute an Affidavit of Late Registration. It is this early date of January 2005 that I became aware of this problem because of the reimposition or typing of October 4, 2004 instead of September 4, 2004 as the actual date of marriage solemnization. Our said representative, Ms. Tumampil, failed and became irregular in her office work since the start of this year (2005) and the consequent need of Spouses ELEAZAR C. SALLOMAN and ELIZABETH L. PUTAN to come to me to refer the problem that problem of presenting before your good office with the month of October superimposed over the month of September as the month of their marriage, which is really apparent in the Certificate of Marriage. This would not have happened if our liaison, Ms. Tumampil, been careful of documents in her hands.
I have to apologize to your good office, as well as, to spouses ELEAZAR C.
SALLOMAN and ELIZABETH L. PUTAN for this kind of irregularity which was kept from me but which was rectified by our liaison officer to make all things legal, but which in effect did not present the truth of the wedding having solemnized in September and not October. This has to be done in order to put things legal in the eyes of the law although not accurate in the dates that our liaison officer made it appear. I have to make this manifestation before your goodself/good office in fairness to Spouses ELEAZAR C. SALLOMAN and ELIZABETH L. PUTAN, who, in fact, carry the burden and trouble of the mishandling of this matter by our liaison officer but kept from me to the prejudice of your good teacher, Ms. Elizabeth L. Putan now Mrs. Elizabeth Putan-Salloman. I just hope and trust this matter would not happen again and again our apologies to this unusual incident by our liaison officer. By the way, I have a son-in-law Engr. LITO MEJIA, married to my daughter, Marites Salvador, whose father, my balae is from San Carlos, Pangasinan. I am just thinking aloud if your goodself or your husband is from that province. I take pride also of my youngest daughter, MAGGIE M. SALVADOR, a 3 rd year High School student of that institution.
