Background
The electrical accident occurred at a residential premises. The deceased
received a fatal electric shock while repairing a 2-gang 13-Amp extension
socket-outlets.
Cause of the accident
Investigation showed that:
a. The cover plate of the 2-gang 13-Amp extension socket-outlets (ESO) had
been removed by the deceased. All the live parts in the 2-gang 13-Amp
ESO were exposed;
b.
The exposed live terminal of the 2-gang 13-Amp ESO was at a voltage of
230 volts with reference to the general mass of Earth; and
c.
The deceased was found bare-footed while working on the 2-gang 13-Amp
ESO prior to the accident.

For this particular case, the deceased should have taken the faulty extension
socket-outlet to the electrical shop for repair by a competent person or should
have engaged a licensed electrical worker to carry out the repair, instead of
doing it himself.
