AFFIDAVIT OF EXPLANATION
(SEPARATION-IN-FACT)
I, MARILOU R. JACOBO, of legal age, Filipino, married and with
residence and postal address at No. 83 Medicare St., SSS Homes, North
Fairview, Quezon City, Philippines, e after having been duly sworn to in
accordance with law do hereby depose and state:
1. That on June 24, 1982, the herein affiant was married to Elmer D.
Jacobo Jr. with one (1) and only son;
2. That after several years of HARDSHIP and IRRECONCILABLE
DIFFERENCES as husband and wife; my husband, Elmer D.
Jacobo Jr., left the conjugal home and never again return to our
family since 2006;
3. That after de facto separation or more than seven (7) years up to
present, the herein affiant does not want to
have any
communication with Elmer D. Jacobo Jr.;
4. That the herein affiant does not want to live with Elmer D. Jacobo
Jr. again for the fact that he was already another family and she
wants and her son to live a peaceful life without him; and
5. That I have executed this affidavit to attest to the truthfulness and
veracity of all the foregoing facts that the herein affiant is already
separated de facto for more than seven (7) years with Elmer D.
Jacobo Jr. and for whatever legal intent and legitimate purpose this
may serve.
Further affiants sayeth naught.
IN WITNESS WHEREOF, I have hereunto set my hands and affixed my
