JOINT AFFIDAVIT OF LEGITIMATION

We, ROBERTO P. RIOJA JR. and ROVELYN G. RIOJA, husband and wife
respectively, Filipino citizens, both of legal age, and residents of Brgy. San Jose Sur,
Agoo, La Union, Philippines, after being duly sworn to in accordance with law,
hereby depose and state:

1. That we are the parents of “GINA CARLA G. RIOJA” who was born on
April 29, 2014 in Agoo, La Union; A copy of her birth certificate is
hereto attached as Annex “A” and shall form part hereof;

2. That our daughter “GINA CARLA G. RIOJA” was born out of wedlock;

3. That at the time our daughter was conceived and born, we both possessed
no legal impediment to marry each other;

4. That on October 10, 2019 we got married; A copy of our marriage

contract is hereto attached as Annex “B” and shall form an integral part
hereof;

5. That by reason of our subsequent valid marriage, the above-named child

shall ipso facto be LEGITIMATED pursuant to the provisions of the New
Family Code of the Philippines, as amended by RA 9858;

6. That our daughter shall now be known as “GINA CARLA G. RIOJA”;

7. That we are executing this Affidavit to attest to the foregoing facts, for
purposes of complying with the requirements in applying for the
legitimation of the birth status of our daughter by subsequent marriage,
for proper recording of such legitimation in her civil registry document,
and for all legal intents and purposes it may serve.
