HOME VISIT REPORT

Name of Employee: Jowen Antonio V. Tandoy


Address: 38 A Mabini St, Arty Subdivision, Karuhatan, Valenzuela City
Diagnosis: Acute Gastroenteritis t/c Amoebiasis
Travel time: 8:30 am- 1:30 pm
At about 8:30 am, I went to Mr. Tandoys house through a taxi cab ride.
Upon arrival at 10:00am, I did initial assessment (Temp: 36 degrees Celsius)
and asked his condition. He told me that he is still experiencing daily LBM with
watery stools and stomach cramps. I then asked him for his documents supporting
his recent absences and he gave me his medical certificates and prescription
papers.
According to him, his condition worsened on May 11, 2016 (Wednesday)
wherein he only worked for half of his shift.
May 11: Half day work, experienced severe abdominal pain followed by several
rounds of LBM with watery stools
May 12: The physician recommended him to be confined to the hospital but they
refused. He was given 3 days rest (May 12- 14) He was given the following
medications: Ciprofloxacin, Omeprazole, Buscopan, Hidrasec
May 13: He came back again at the hospital for check-up (out patient)
May 16: He had a follow up check up again at FUMC and was given another 3 days
rest (May 16-18). The diagnosis is as follows: t/c Acute Gastroenteritis, t/c Irritable
Bowel Syndrome
May 17: He was rushed to the ER/Out-patient at about 10-11pm and undergone
fecalysis exam.
May 18: He came again for a follow up check up and the physician added dosages
of Buscopan to his prescription.
May 19: He went for a follow up check up again and was given Lansoprazole,
Riflaximin, Gaviscon, Eldicot. He was also advised to undergo endoscopy on
Saturday ( May 21, 2016)
He also told me that his doctor is still unsure of his condition. He was required
to undergo endoscopy to rule out if the LBM was triggered by gastric acids, and to
check for Ulcerations. The physician also wants to consider the fact that he has a
history of Colon Cancer in the family. For Endoscopy procedures, the employee will

be required to have NPO (no food intake for 4-8 hours) for preparation. I told him to
inform Sir Yves for further recommendation and evaluation of his doctor.
I left the house at about 10:48 am.

Diagnosis: Hyperemesis Gravidarum, Threatened Abortion
Travel time: 1:30 pm- 2:00 pm

At about 1:45 pm, I entered the house of Ms. Balbaguio to assess her
condition. She said that she had already told Maam Ghie Lara this morning about
her plans to resign.
In her medical certificate, it is indicated that the doctor was requesting for an
indefinite leave due to problematic pregnancy. According to her, she was advised
complete bed rest. Thus, she was not allowed to travel anywhere because of her
condition. She informed me that she was felt dizziness, headache and abdominal
pain whenever she is travelling and that she cannot control her balance.
