Conception Grande Naga City, after having been duly sworn to in accordance with law,
hereby depose and declare;
1. That I am the Manager of the JANSHA CONSTRUCTION & SUPPLY, a sole
proprietorship duly registered in accordance with law with principal office at
Phase 2, VGH, Conception Grande, Naga City
2. That I have inspected the site for the above mentioned Contract at the above
stated location; and
3. That I am making this statement as part of the requirement for the Technical
Proposal of the JANSHA CONSTRUCTION & SUPPLY for the said contract
