Unsafe Bag Lifting

The Alerts are provided to all Drilling and Workover Operations Foremen and Liaisonmen so that we all learn from the
experiences of others. The knowledge of this incident could prevent similar ones from happening in the future.
Saudi Aramco policy requires the information in this Alert to be communicated to all crew members and documented on
morning reports.

What happened?
On two separate recent rig visits, LPD observed that contractors were picking up
two big bags at a time with the crane. This is an UNSAFE lifting practice and was
noticed again at another different site yesterday.

Consequences:
High potential for fatality, serious injury or property damage.
Why did it happened?
Crew handling the lift trying to complete the task as fast as possible.
Crew unaware that the practice is unsafe.
What should be done to prevent recurrence?
Re-train supervisors and crew on the correct lifting practices for big bags and
strictly only one big bag can be lifted at a time.
Follow the attached Flexible Intermediate Bulk Container Association
(FIBCA) handling guidelines:
o Handling big bags with cranes/hoists, forklifts, etc.
o Storage (weather, etc.) & transportation of big bags (adequately secured, etc.)

Report any unsafe lifting acts or conditions to your supervisor.
