I. Subject
a. Name: J.S
b. Age: 3 Years Old
c. Race, ancestry: White
d. Gender: Male
II. Information Source
I used the 3 year old checklist and the childs observations in the class as well
as a small interview with the mother. I asked the mother some questions
about their lifestyle and about herself. I learned more about this child and the
reason why he had great outcomes on the checklist. The mother works a lot
to help her child achieve in the academic level.

III.

Background Information
a. J.S. is a 3 year old that lives with the mom, and mothers parents. J.S.
gets dropped off by the mother everyday and gets picked up by her
except on Fridays; grandpa picks up. By interviewing the mother, I know
that she has a part time job and on weekends she works on cosmetology.
The childs father in not present and the mother recently changed the
childs last name to hers. The mother has associates in business and a
cosmetology degree.
b. J.S. is a healthy child and never comes in sick. The child has blue eyes
and dark blonde hair. J.S. is a bit picky with food but often bring in
snacks. The child is constantly smiling and is very cheerful.

c. J.S. attends to class everyday and rarely misses a day. The child
participates in all the classroom activities and loves reading time. The
child gets along with most of the children from the class but tends to sit
next to the girls.
d. According to the assessment tools, J.S. is typically developing; the child
catches up to the task quickly. When the child is outside and it is time to
go inside the child quickly runs to pick up all the toys and stands in line.
The child is always aware and prepared for what comes up. At the childs
age, J.S. is a quick learner and has achieved many goals.

IV. Developmental Performance

a. J.S. main interest areas are dramatic play and the train table. The child
likes to play in the dramatic play area with the girls and loves to play vet.
J.S. likes to wear pink gloves and just walk around and pretend to be a
princess. In the train station, the child plays with A.P. and spends much
time there.
b. J.S. areas of limitation are the science area and manipulative area. J.S.
does not find those areas interesting. The child says, I dont like puzzles
theyre boring, or animals are better in the kitchen area. The child does
enjoy sitting down and counting bears or matching shapes.
c. J.S. personality is very bright and cheerful. The child has a smile on his
face most of the day. J.S. never cries for any little thing. J.S. gets happy
with a hug and loves to have my attention.

V. Conclusion and Recommendations

a. J.S. is a great child and very open to his surroundings. J.S. never fights or
hits other children in the class. Time to time he does like to wrestle with
the boys but never cries or hits. The child never raises his voice at me in a
disrespectful way. He knows how to treat his elders and friends. When he
does not like something he stays quiet or tries to find interest in the
subject.
b. The child does need some help in some of the patterns matching and
identifying some opposites as well as identifying between short/small and
tall/big. I did print out many shapes and copies of them so the child could
practice. The mother has helped a lot and has bought the child a shape
book. I also made a chart to help him understand how items can be short
and big or tall and small.
c. In overall, I recommended J.S. mother to keep up the great work of
helping him learn and trace his shapes. So far, he has learned to trace a
triangle. He is a quick learner and I know he is in a great track. I would
recommend his mother to let him do more on hands activity with the
shape tracing. For example, on a technology device she can download a
learning app that includes shapes.
