Subject: Media Center self-evaluation

I have completed a self-evaluation of the Media Center using the GA DOE 2015 Library Media Program
Evaluation Rubric. There are six categories and within those categories there are multiple target
indicators. Each indicator was graded by level of proficiency. The levels are basic (B), proficient (P), and
exemplary (E).

The East Laurens Media Center scored Basic in 5 indicators, Proficient in 5 indicators and Exemplary in
11 indicators. I am overall satisfied with the score, but I have clearly identified areas where we could
improve. I am proud to say we are Exemplary in all indicators within the facilities, access, and resource
category.

I have attached the completed rubric along with an action plan to help us move our basic scores to
proficient, and proficient to exemplary. Please review these documents and contact me with any
