WAIVER
I am allowing my son/daughter __________________________________ with student number __________ to join
and participate the ASEAN Undergraduate Chemical Engineering Education Summit to be held on September 18 22, 2016 at Manila, Philippines.
Together with my child/ward, I know the organization and its officers, faculty and staff are expected to exercise
impartial diligence legally required for the safety and well-being of my child/ward within the duration of the activity and
supervisory vicinity of the activity as stated above. This legal diligence include oral and written instructions, whether
given before or during the activity, that if followed, would ensure the safety of my child/ward. If my child/ward
disregards or fails to follow these instructions or should act on his/her own accord in contravention to the guidelines
set forth and given. I, together with my child/ward shall have no claims against the organization, faculty, staff-incharge, etc. should any damage be caused or liability be incurred to property or person.
I hereby agree to the above specified provisions.
