I,Mary Grace Otic, of legal age, Filipino, single and a resident of Ozamis City,
after having been duly sworn to in accordance with law, depose and state:
1. That I was present and personally saw that accused, Karissa Villondo,Julius
Apao,Daryl Mabia and Jocelyn Ganade, committed the crime of Illegal Drugs.
2. That (State other circumstances and the relation either to the victim or accused or
both);
3. That they sale illegal drugs.
4. That when I buy in the sari-sari store I saw them buying illegal drugsshabu on the
same store.
5. That I executed this affidavit to attest the truthfulness of the foregoing facts and to
