AFFIDAVIT OF PARENTAL CONSENT TO

MARRIAGE
I, Ceferino B. Garay Jr. of legal age, Filipino citizen, single and
presently residing at #3 Earth St., Sunnyside Heights Subd., Batasan Hills,
Quezon City, after having been duly sworn to in accordance with law, depose
and state THAT:
1. I am the father of Junezza Cedeo Garay born in Pasig City on August 02,
1995;
2. Our child is intending to contract marriage with Merwin Fiel Lorica
Cristobal in Quezon City;
3. I am giving my consent to our child, Junezza C. Garay, marrying said Merwin
Fiel L. Cristobal;
4. I am executing this Affidavit for the purpose of informing the authorities
concerned
of
the
veracity
of
the
foregoing
facts
and
for
whatever legal purposes it may serve.
