Each DCIS student must complete 5 hours of school service each semester. The purpose of this approval form is to provide an
opportunity to reflect on your contribution to the DCIS community and to track your progress toward meeting your program service
requirements. You may use the following questions as guidelines as you write a solid paragraph describing how this service
connects to specific outcomes in the four Global Leadership domains:

How did you identify this opportunity for school service (did someone recruit you, did you figure it out yourself, was it
part of a club, etc.)?
Describe how you worked collaboratively with others to perform your service.
What evidence can you present to demonstrate the impact of your service on our community (school, local, regional,
global)?
Reflect on how you changed as a result of your service.

Description of how this service connects to outcomes in the Global Leadership Domains:
Kelsey recruited me to help her with everyday tasks while she went to meetings and took care of planning events.
I worked collaboratively with Kelsey and other volunteers to figure out what needed to be done and complete each task.
All of the invitations and information for the Art Auction, Christmas Store, Spring Events, and other events put on by JS
were sent out with my help. I now realize patience is a virtue and packing envelopes is tedious work. This is Take Action
because I was able to identify people to collaborate with and create my own opportunities for both individual and
collaborative collaboration to improve the lives of Joshua Station families. I am also reflecting on my capacity to give
back to the local community.
