Each DCIS student must complete 5 hours of school service each semester. The purpose of this approval form is to provide an
opportunity to reflect on your contribution to the DCIS community and to track your progress toward meeting your program service
requirements. You may use the following questions as guidelines as you write a solid paragraph describing how this service
connects to specific outcomes in the four Global Leadership domains:

How did you identify this opportunity for school service (did someone recruit you, did you figure it out
yourself, was it part of a club, etc.)?
Describe how you worked collaboratively with others to perform your service.
What evidence can you present to demonstrate the impact of your service on our community (school,
local, regional, global)?
Reflect on how you changed as a result of your service.

Description of how this service connects to outcomes in the Global Leadership Domains:

I have been volunteering at the animal shelter down the street called MaxFund for many years, off and on. Recently, in the
second semester of passages, I came up with the idea to do my activity passage on this animal shelter. I was equipped with
time to devote to volunteering during my free period, and I started going at least twice a week for the duration of the last
class period. I would either walk dogs, or take them out to play/ socialize in the dog run. I loved doing this and plan to
continue to expand my volunteer work, and to continue volunteering regularly.

difference, and kept up with documenting it for my passage. >
