Client Centered Care

Did I keep in mind the original concern as I solved the problem?


(Describe).

During assessment, I was concerned when patient reported

decreased sensation to left foot. Patient denied sensation to left foot

when I touched it. Patient had a wound vac to left ankle secured

with ace wrap dressing. I was worried and surprised. It was the first

time a patient reported decreased sensation to me. I was glad that

the nurse was with me at that time. It was hard to assess when

decreased sensation started because she was deaf. It sounded like I

was yelling at her.

Surgeon was notified immediately, and wound vac was

removed. When I reassessed patient after removal of wound vac, I

remembered my original concern which was decreased sensation. I

was hoping that this time she would tell me she can feel my touch,

and I was right. Patient admitted she could feel sensation when I

was touching her toes. When I applied wet to dry dressing to her left

leg, I documented the small amount of sanguineous drainage and

that patient was able to feel sensation when I touched her feet even

after wrapping her ankle with ace dressing.

As a registered nurse in the future, I can apply this great

experience when assessing my patients especially post op from

surgery who are at risk for DVT and decreased perfusion.
