Creating a new accident notice

Creando un Nuevo aviso de accidente.

PPT 9

Start creating a new accident notice by selecting Accident from the New
notice box in the top left corner.

The Accident notice: basic data window appears.


- Enter the required fields
- The more detailed the description you give, the faster the corrective action
can be started.

Continue completing the accident notice by selecting Next

Fill in all the next complete the accident notice and then click Save and exit.
The program then creates an archiving code for the form.
- Save and exit to complement and edit the notice until the accident
investigation has been launched.

la caja Nuevo aviso ubicado en la parte superior izquierda ( de la esquina
superior izquierda).

"Guardar y salir". A continuacin, el programa crea un archivo cdigo para el

Accident notice Title data


Enter the date of the incident either by typing the date as dd.mm.year or by placing
the cursor on the calendar window and selecting the date from the calendar. Enter the
time of the incident as hh:mm.
To start creating a notice, enter the title data. This data is later shown in the accident
notice browsing window. The
title data should describe the accident as clearly and specifically as possible. For
example, Fingers got stuck into
bending machine (the title is used as a search criterion when searching and browsing
documents).

Injured person
- Who is the injured person?
Select My self if the injured person is the person submitting the accident notice.
Select Personnel if the injured person is a Metso employee but not the person
submitting the accident notice.
Select Subcontractor if the injured person is someone other than a Metso
employee.

- Select personnel
- Search for the injured person from person selection
- Click on the person icon
- Search for the injured person
- Type the lastname and select find...

Accident notice - Place of incident


Retrieve the place of incident tree from the Place of incident tree.
Click on the organization icon next to Place of incident tree to open
the list view:
- You can choose the unit by clicking on the desired organization.
- Always choose the place of incident from the sub-branches on the lowest level under
the main branch!
In the Detailed place of incident field, describe the place of incident more
specifically, e.g in front of the main port

Accident notice - Description


Describe as specifically as possible
- The Description section should describe the accident in more detail. When
completing the fields, please note that the text typed in the fields will be accessible to
all program users.
Do not use personal data, e.g. the name of the injured person in the description!
- In the Description of incident field, accurately describe the progression of events,
including the events immediately preceding the incident. Also record the
environmental conditions, any objects that should not have been at the place of the
incident and whether the protective devices were on/off.

Accident notice - Immediate corrective


actions/proposals for improvement
In this section, record any immediate corrective actions found necessary during
the investigation of the accident, actions such as use of personal protective
equipment, repair or prohibition of use of equipment, or sanding of roads. Example
related to the accident above: Marking the holder with red/yellow tape, use of
helmet while working inside the calender frame.

Accident notice - For the information of the


following
Under For the information of the following, you can send the document for the
information of the specified persons.
If the notice is filled in by a person other than the employees supervisor, the
document should be sent at least for the information of the supervisor and the
occupational safety delegate/representative.
The document is automatically sent to the responsible person specified by the plant
who appoints an investigator for the case as well as to the persons with investigation
rights for the level of the place of incident in question.
To select the persons to whom you wish to send the document, click on after the
text CC to the following. The following window opens. You can search for the person
by using his or her last name and clicking on find. First click on the name of the
desired person and then on Accept at the bottom.

Accident notice - Attaching a file

You can attach files related to the incident, such as photos or other documents, to
the notice in the Attached files section.
Click on Browse to retrieve files from your computer. Type a description for the
file if necessary (e.g. describing the photo).
Select Attach file.
Please note that the file is added to the notice only after you have clicked on Attach
file.

Accident notice- Type of incident
From the drop down list, choose where the accident occurred
- This information is important for statistical reasons
Severity
- Under Severity, select the level of severity of the injury
from the pull-down menu.

PPT 20

Death : An accident resulting in death.

LT4 : Lost Time Injury, LTI. An accident resulting in the injured persons absence from work or
inability to perform normal work tasks for a period of four or more days, calculated from the day
after the accident. The days are counted as calendar days.

LT1: Lost Time Injury, LTI. An accident resulting in the injured persons absence from work or
inability to perform normal work tasks for a period of one day, calculated from the day after the
accident. The days are counted as calendar days.

MEDICAL TREATMENT : Accidents in general requiring treatment by a physician, registered


medical professional, or in the case of an emergency, a non-professional (non-medical person).
Medical treatment does not include first-aid cases (one-off examination, treatment and follow-up
examinations of small scratches, burns, wounds caused by chips of material and other similar
injuries), even if carried out by a physician or a registered medical professional.

RESTRICTED WORK: This category is used if the patient/injured person is unable to perform
his or her normal work tasks but can perform restricted tasks and the accident does not result in
absence due to sickness.

FIRST AID: Any one-off treatment or follow-up examination of a small ulceration, scratch, burn,
wound caused by chips of material or other similar injury that does not usually require medical
treatment. These are considered first aid, even if carried out by a physician or registered
medical professional.

Environmental damage :Incidents resulting in emissions into the environment (into the ground,
water or air).

Near miss: An incident that in slightly different conditions might have resulted in an accident
and/or material damage/loss. Identifying near miss cases requires effort. A near miss is reported
as an observation.

Observation An observation relating to hazardous conduct or the dangerously poor condition of


equipment.
