Start creating a new accident notice by selecting Accident from the New
notice box in the top left corner.
Fill in all the next complete the accident notice and then click Save and exit.
The program then creates an archiving code for the form.
- Save and exit to complement and edit the notice until the accident
investigation has been launched.

Injured person
- Who is the injured person?
Select My self if the injured person is the person submitting the accident notice.
Select Personnel if the injured person is a Metso employee but not the person
submitting the accident notice.
Select Subcontractor if the injured person is someone other than a Metso
employee.

- Select personnel
- Search for the injured person from person selection
- Click on the person icon
- Search for the injured person
- Type the lastname and select find...

You can attach files related to the incident, such as photos or other documents, to
the notice in the Attached files section.
Click on Browse to retrieve files from your computer. Type a description for the
file if necessary (e.g. describing the photo).
Select Attach file.
Please note that the file is added to the notice only after you have clicked on Attach
file.

Accident notice- Type of incident
From the drop down list, choose where the accident occurred
- This information is important for statistical reasons
Severity
- Under Severity, select the level of severity of the injury
from the pull-down menu.

LT4 : Lost Time Injury, LTI. An accident resulting in the injured persons absence from work or
inability to perform normal work tasks for a period of four or more days, calculated from the day
after the accident. The days are counted as calendar days.
LT1: Lost Time Injury, LTI. An accident resulting in the injured persons absence from work or
inability to perform normal work tasks for a period of one day, calculated from the day after the
accident. The days are counted as calendar days.
RESTRICTED WORK: This category is used if the patient/injured person is unable to perform
his or her normal work tasks but can perform restricted tasks and the accident does not result in
absence due to sickness.
FIRST AID: Any one-off treatment or follow-up examination of a small ulceration, scratch, burn,
wound caused by chips of material or other similar injury that does not usually require medical
treatment. These are considered first aid, even if carried out by a physician or registered
medical professional.
Environmental damage :Incidents resulting in emissions into the environment (into the ground,
water or air).
Near miss: An incident that in slightly different conditions might have resulted in an accident
and/or material damage/loss. Identifying near miss cases requires effort. A near miss is reported
as an observation.
