Remarks

Fact of the case:

At about 2:15 PM of Oct 1, 2017 driver of V-1 and V-2 appeared before this office and reported the above caption
incident for investigation. The undersigned took cognizance of the case.

Investigation conducted by this office revealed that V-1 Ford Mustang with CS. No. SJ-0218 driven ENRICK DAVE y
GUDELANO and V-2 Chevrolet Colorado with plate No. AS-2345 driven by EDYLENE y AQUINO were both traversing southbound
lane of Maria Orosa. When these vehicles had reached a certain portion particularly at the corner of Kalaw Ave. in Malate, Manila. V2
driver veered to the right in avoiding hitting its front portion by another vehicle. As a result, the left side middle portion of V1 was
hit/bumped by V2. That due to the force of impact said vehicles incurred un-estimated amount of damages, none was reported injury.

Action taken: Both drivers were investigated and advised accordingly. This report is being issue upon request of said parties for
whatever legal purpose it may serve.
