Survey Questions on Bullying Among

Teenagers

Dear Respondent,

We are Grade 11 senior high students of Silliman University. We are writing to

ask for your participation in a survey conducted about Bullying among
Teenagers for our EMPTECH activity. We are inviting you to take part in our
survey by answering the attached questions.

This survey is a key element in our advocacy project about bullying among
teenagers; its effects, its types, and how to stop it. We are seeking your inputs on
this matter.We would be very grateful if you could take even just 5-10 minutes of
your time to complete the questionnaire.There is no compensation for
responding nor is there any known risk, but your responses are a big help to our
survey study. If you choose to take part in this survey, your answers are
completely confidential and will be released only as a part of group summaries.
