The Epilepsy Foundation of Florida is a support group where one can get in contact with other
parents who are experiencing or have experienced what the family is dealing with right now.

FAMILY INTRODUCTION

Neil and Barbara live in Miami, Florida. Neil grew up in Miami and spent the majority of

his childhood on the beach. Barbara grew up in Georgia. Her parents passed away before she

was married but when they were alive she was very close with her mom. For the first few years

of their marriage, they would rely on Neils family for support and babysitting but his parents

recently moved across the country. Barbara has one sister, but they are not close. Neil is a

biology teacher at the local high school. He is kept very busy and often works late, which leaves

Barbara to take care of things at home. The Jones family make $47,000 a year off of Neil's

teaching salary and are considered to be in the middle class. Barbara currently stays at home

with their three kids and will occasionally style hair for close friends and family. Their

neighborhood is located just over a mile away from Miami Senior High School where Neil works

and less than a mile away from Auburndale Elementary where Lily and Mason attend school.

The school district they are in is called Miami-Dade County which is the fourth largest school

district in the United States.

Neil and Barbara have three children. The two oldest children are Lily, who is 7 years old

and is in 2nd grade and Mason who is 5 years old and is in kindergarten. Aiden is the youngest

child. He is two years old and has been recently diagnosed with a tonic clonic seizure disorder.

Up until his first seizure Aiden had been typically developing but now he appears to have lost

developmental milestones. He is unable to sit, walk, or talk.

As a family the Joness love to spend time outside. On Sundays they love to order in

food and watch a movie together. They play outside together and make up their own games.

Lily loves to dress up and also get dirty. She is a master at playing pretend. She loves to read

and do DIY science experiments. She loves to learn and loves to go to school. She loves being

outside and adventuring, but also knows how to entertain herself inside the house. Mason is

really adventurous. He loves being outside and loves to study nature and bugs. He also loves to

ride his bike, and especially loves to race. Aiden is the quieter of the three children. He likes to

observe the other children playing. His favorite toys are balls, and he loves to ride his bike, and

play with anything he can squish in his hands. During the day, when his siblings are at school,

Aiden watches a lot of TV.

FAMILY STRENGTHS

Barbara is a very loving and caring mother. She is always there for her children when

they need her. She loves taking her children on outdoor adventures such as biking, hiking and

going to the beach. Neil is reserved and usually doesnt say too much. He is always working to

provide for his family. He loves going on adventures with his family just like his wife. Lily is more

like her mother, very happy and outgoing. At school Lily has lots of friends. She isnt afraid to

talk to anyone. Mason is more like his father, shy and timid but it doesnt stop him from

exploring. Hes smart and loves study things that he is interested in. Aiden is happy and loves

trying to keep up with his siblings. Aiden isnt shy he will talk to anyone and everyone. Barbara

and Neil usually have a healthy, loving relationship. They have their ups and downs, but they

typically have good communication. When they get into heated disagreements, they try not to

do it in front of their children. They strive to be the best parents that they can be and like to

learn about new ways and the best ways to parent. Lily, Mason and Aiden get along pretty well.

They play outside together and make up their own games. Lily is very responsible for her age

and is moms little helper when she is needed. Barbara and Neil trust her a lot because she has

proven to them that she can follow what they ask her to do.

INFORMATION SUPPORT

1st Family Concern/Priority: Barbara wants to know how these seizures will affect her son's brain

development. She is afraid that the seizures will put him behind his peers developmentally,

which will effect him both academically and socially. Barbara wants to know about other

families who have a child with this seizure disorder. How have their children done in school?

Have they been successful in adult life?

Current Resource/Information: Information from the doctor

Outcome: The family will have a deeper understanding of his condition and know the effects it

will have on his brain development.

Suggested Action: We will immediately make a referral to the public health nurse, who will

make a home visit to provide information and support to Barbara regarding tonic-clonic

Program information The Epilepsy Foundation of Florida is a state resource. Their Case

Management Program provides individuals with seizures and their family with Individualized

Plans of Care tailored for the specific needs of the client and family. Every client is assigned to a

case manager who will walk them through the process and find them the help they need.

Applications/Forms You must call their toll free number or visit them to get the information

needed. Or you can send a message to them on their site and they will email you the

information needed.

Rationale: The family can call and get the information they need, apply, and have someone

come out and help them. This would help the family gain information on Aidens condition and

what they can do to help. It offers them a professional that will be there to help and guide

them.

2nd Family Concern/Priority: Barbara and Neil want to know if they qualify for medicaid and

government assistance. They do not have a lot of money and are concerned about how they

will pay medical bills.

Current Resource/Information: The insurance they currently have is through Neils job.

Outcome: Barbara and Neil will receive government help for paying medical bills.

Suggested Action: Barbara and Neil will apply for medical assistance and hope to qualify.

program for children ages 1 through 4. Low cost healthcare.

Applications/Forms Need to have familys most recent tax return or wage and earning

statement (W-2 form) or Current pay stubs (covering the last four weeks) and your childrens

social security numbers or the date applied for if you have not received a social security card

Rationale: Barbara and Neil have insurance through Neils job but they are still struggling

financially. They will most likely not qualify for medicaid for the whole family but could

potentially qualify to get medical financial aid for Aiden through the Kidscare program. This

would relieve a lot of stress on the family and allow them to continue to live comfortably in

their home and neighborhood.

FORMAL SUPPORT

1st Family Concern/Priority: Where Aiden has tonic-clonic seizures he has lost his developmental

milestones. Barbara is worried that Aiden will never get these milestones back that he has lost.

Barbara is curious if a developmental specialist will be able to help Aiden gain back his

milestones.

she has two older children.

Outcome: Barbara and Neil will get help for Aiden in getting his milestones back.

Program information - Since 1993, the Child Development Center has provided the highest-

quality diagnostic and early intervention services to children and families. The Center evaluates

infants and children for potential problems that might require medical treatment, therapy

services or parent education.

Policy Information - Visiting hours are from 9a.m. - 9p.m. daily. After 9 p.m., all patients and

visitors must enter and exit through the Emergency Center. The main lobby will be closed

Rationale: With a developmental specialist Barbara and Neil will be able to see how they work

with him. They can learn of things that can help with his milestones and gaining them back.

Barbara will feel better by just having a specialist being able to help Aiden in the milestones

that he is struggling with.

2nd Family Concern/Priority: Barbara and Neil have been fighting lately and their children are

struggling in school. Barbara wonders if they got some help for her and Neil and for the children

in school that it could help.

Current Resource/Information: Barbara has talked to the older childrens teachers about their

grades and what they are struggling with in school.

Outcome: We will find academic and emotional support through the school district to help the

older children and to take stress off of Barbara and Neil.

Suggested Action: Refer Lily and Mason to counseling services provided through the school.

Program information - School Counseling services are provided by school

counselors at every school PK-12 and adult/vocational/technical education centers. School

counselors assist students in the developmental acquisition of knowledge and skills that will

help them reach their academic, personal/social, health and career/community potential.

Masons teachers to determine eligibility for counseling services

Rationale: Lily and Mason are struggling in school because of the stress at home. They need

support in school both emotionally and academically. By going to counseling they will be

provided with the emotional support necessary to help them achieve academically. This will

help relieve stress on their parents because they will not have to worry about them as much.

The counseling services through the school are free and will not be a financial burden on the

family.

INFORMAL SUPPORT

1st Family Concern/Priority: Barbara is feeling lost and alone. Her parents passed away years ago,

she isnt close to her sister, and Neil and her have been fighting. She wonders who she can turn

to for support during this hard time.

Current Resource/Information: Barbaras only support right now is her immediate family, but Neil

and her havent been getting along.

Outcome: Barbara will find someone that she can talk to about what shes going through.

Suggested Action: Barbara will contact the local Epilepsy Foundation in Miami and get access to

their support groups they offer so she can start talking with others that are experiencing similar

things as her.

Program information - The Epilepsy Foundation of Florida provides services to individuals with

epilepsy and their families including support groups.

Policy Information - Hours are Monday-Friday 8:30 am - 5:30 pm this foundation takes

donations but dont have any fees that have to be paid to be involved with support groups.

Rationale: Barbara has been through alot and is feel very alone. She doesnt have anyone to talk

to, so contacting the Epilepsy Foundation of Florida to join a support group would be very

beneficial for her. She will be able to learn that shes not alone and there are other moms who

are experiencing or have experienced what she is dealing with right now in her life.

2nd Family Concern/Priority: Neil is having a hard time balancing work and the kids as Barbara has

been driving 3 hours away for Aidens testing. The couple are worried about how they are going

find someone to help watch the kids since Neils parents recently moved across the country.

Current Resource/Information: Barbara and Neil dont currently have a regular babysitter because

Neils parents, who used to watch them, moved away

Outcome: Barbara and Neil will find a regular babysitter that fits their needs.

Suggested Action: Barbara will contact a qualified babysitter she found on a local babysitting site

to learn more about her and decide if she is a good fit for their family.

Psychology, is CPR & First Aid certified, has experience working with children who have special

needs and has been babysitting for over 5 years.

Policy Information - She is available to watch the kids when they get out of school from 3-9pm

M-Th and 3pm-12am on F.

Rationale: Neil is having a hard time juggling work and taking care of the kids with Barbara being

gone often with Aiden for testing. The family could use a nanny to come after school and help

with the kids to relieve some of the stresses that Neil and Barbara are dealing with. Deborah is

very qualified for the job and even has the skills to work with Aiden in his condition.

MATERIAL SUPPORT

1st Family Concern/Priority: Barbara and Neil are worried that Aiden may have a seizure while

sleeping. They are worried they wont hear him and that he be injured badly. Barbara wants to

find out if there are materials they can implement in the home to make it safer for Aiden.

Current Resource/Information: Barbara and Neil dont have a way to keep an eye on Aiden while he is

sleeping if he has a seizure

Outcome: Barbara and Neil will find a mat to place by Aidens bed that will alert them if he has a

seizure and falls off of the bed while he is sleeping.

Program information - The MP5V2 utilises patented sensor technology to provide reliable bed

seizure alarm detection from a wider range of seizure types, bed types, and patient body

weight. A high specification microprocessor continuously monitors the patients bed, analysing

detected movements. Suitable for all age groups from 5lb baby to adult. Extended sensitivity

adjustment for memory foam mattresses.

Rationale: Barbara and Neil wont have to be worried while Aiden is sleeping after getting a mat

alarm to put under his bed while sleeping. They will be able to monitor him better during the

night. If he does have a seizure in the middle of the night they will be able to wake up and be

with him while he is having a seizure.

2nd Family Concern/Priority: The family is struggling to pay for the travel and housing when going

to and from the hospital. They are spending too much money on hotels and gas to and from the

hospital and hotel.

Current Resource/Information: Barbaras only knowledge of the Ronald McDonald house is what

she has heard from other mothers at Nemours Hospital.

Outcome: The family will be able to save money on housing and food while in Orlando, and will

be able to save and/or put it towards other needs. It also makes the family close to the hospital.

Suggested Action: Barbara will immediately apply for RMH which will save their family money

and allow them to be close to Aiden while he is in the hospital.

Resource Type and Name - Ronald McDonald House

Program information - The Ronald McDonald House is a national resource. While your child is at

the hospital getting treated you can stay overnight in the Ronald McDonald House so you dont

have to pay for a room. It is asked that families donate $20 a night, but no one is ever turned

down for financial issues. RMH provides transportation to and from the hospital, transportation

to and from a nearby retail complex, lunch and dinner provided by community volunteers,

access to a community pantry and refrigerator with donated food.

Policy Information - Must be a patient at an Orlando Hospital. Only family members involved in

the daily care of the patient may stay in the house.
