SUBJECT: AUTHORITY OF THE FOLLOWING PDOHO DAVAO DEL SUR

PERSONNEL TO CONDUCT CONSULTATIVE MEETING WITH LCES,
NGOS AND PROVIDERS ON SDN PARTNERSHIP AGREEMENT AT
HOTEL DE CRISBELLE ON JUNE 23, 2017.

The following PDOHO Davao del Sur personnel, this office, are hereby authorized to conduct a
consultative meeting with LCEs, NGOs and providers on SDN partnership agreement at Hotel
de Crisblle on June 23, 2017, to wit;

2. SALVACION D. RODRIGUEZ, RN, MPH - DMO IV
3. METRIA P. BRAGAS, RN, MAN - DMO IV

All expenses incurred shall be charged to PDOHO Davao del Sur Funds, subject to its
availability and usual accounting and auditing rules and regulations.
