PERSONAL
Provides direct care to patients undergoing hemodialysis based on the rules and regulations established by
Department of Health and approved company and procedures
Checks and prepares all materials needed and ensure it is available before the start of hemodialysis
Reads and carries out doctors’ orders and ensure that dialysis is performed accordingly as per SOP
Assess patient using the nursing process before, during and after hemodialysis.
Explains hemodialysis procedure, secure consent and does pre-dialyisis physical assessment.
a) Performs proper cannulation and observe aseptic technique during the procedure.
b) Carries out dialysis prescription, checks good patency and access, extracts pre and post dialysis.
c) Monitors and observes patients for possible complication during the physical assessment.
d) Reminds patients about care and schedule of next dialysis.
e) Records and reports condition and progress of patients under care.
f) Prepares and administers medication and other treatments as indicated.
g) Gives health teachings to patients and relatives.
h) Ensure the safety of patient during procedure.
Initiates and performs Cardio-Pulmonary Resucitation (CPR) in the event of cardiac and/or pulmonary arrest
and sees to it that all activities are supervised, organized and documented.
Gives proper endorsement to the incoming shift.
Does necessary referrals to other departments concerning patients’ needs.
Updates and maintains confidentiality of records and other information regarding patients ‘care.
Observes the lines of communication and appliers appropriate communication techniques in reporting,
recording and relaying information and data.
Inventories e-cart medicines and ward stocks at the end of every shift.
Provides clean, safe and therapeutic working environment.
Responsible for maintenance and safe keeping of equipment and supplies in the unit.
Accomplishes daily and weekly reports of the unit.
Attends meetings, conferences and other development program.
Upholds and implements hospital policies, rules and regulations.
Performs other tasks assigned by the supervisor, Medical director and Hospital Administrator.
Passed the Licensure Board Exam for Nurses 75.80%. July 2, 2018

With almost 1 year of experience as Hemodialyis Nurse- Local. Familiar in handling hospital apparatus
and or equipments such as: Hemodialyis Machine (Fresenius 4008A and Fresenius 4008S), Cardiac Monitor, Suction
Apparatus, Built-in suction apparatus, Oxygen Nebulizer, Crash Cart with Defibrillator,Infusion Pump,
Electrocardiogram, Treadmill Exercise Test (Stress Test), 24-Hour Holter Monitoring, and 24-Hour Ambulatory BP
Monitoring.
