Incident & Injury 1: Prevention:

Lost 3 fingers from pizza roller  Turn off machine before cleaning
Worked at Pizzeria  Ask for help from co-workers
Saw pizza dough on the roller and cleaned it by hand  Used tools to clean the pizza roller

Partially paralyzed with a wheelchair  Move out of the dangerous space
