Incident & Injury 1: Prevention:

Jennifer Fourchalk reached into a dough machine in
order to clean some dough that she found on the chain;
however, her fingers got caught in the chains and
caused her to lose three fingers
 The dough machine was turned off
 She had a better nights sleep, or called the day off
 If there were more protection on the machine to prevent
people sticking their hands in the chains by accident

Incident & Injury 2: Prevention:

Michael Lovett got his leg caught on the chain of a
running machine while trying to get down. It then held
onto him and sucked his leg into the roll feeder. The
machine was only turned off after he already got his leg
sucked in up to his thigh.
 The heavy-duty machine is only operated by experienced
workers, who have worked with smaller machines
previously.
 The chain should have been covered and not accessible to a
worker while the machine is running.
 An emergency stop button is put in place near the opening
to the press to be able to stop/turn off the machine.

Incident & Injury 3: Prevention:

John Higgins broke his back at age 16 when he put a  There was more supervision on new workers

 The load was more tightly secured.
 The other worker was watching where the boy was before
he dropped the load.
 The forklift had sensors installed to prevent it from lowering
anything while someone is in the way.
