INFORMATION SHEET FOR FILIPINO OR RESIDENT ALIEN
SCHOOL YEAR 2018-2019

I. GENERAL REQUIREMENTS FOR APPLICATION:

a. Accomplished at the official website www.uerm.edu.ph
b. Original /Certified True Copy of Transcript of Records
c. Certified copy of AB/BS Diploma/Certificate of Candidacy for Graduation
d. Original Certificate of Good Moral Character and 2 (two) letter of recommendation from 2 former
professors.
e. National Medical Admission Test (NMAT) Result
VALID NMAT: October 2016, March 2017, October 2017, and if there are slots
available, March 2018.
f. Birth Certificate authenticated by National Statistics Office (NSO)
g. Marriage Certificate, if married
h. 2x2 Pictures ( 2 Pieces Colored White Background)
i. Application and Processing Fees: Php 3,000.00(NON-REFUNDABLE/NON-TRANSFERABLE)

II. DEADLINE FOR SUBMISSION OF APPLICATION IS JANUARY 15, 2018.

List of Accepted Applicants will be posted on the Bulletin Board of the Office of the Registrar.

III. MINIMUM REQUIREMENTS:

a. Applicant must be a holder of baccalaureate degree;
b. Interview upon submission of required documents (for scheduling); and
c. Applicant must have taken the National Medical Admission Test (NMAT) within the
validity period and must meet a preferred NMAT percentile score of 75. Applicants
who wish to improve their NMAT percentile score may retake the test.

IV. REQUIREMENTS FOR ENROLLMENT:

a. Complete Official Original Transcript of Records
b. Certificate of Graduation / Copy of Diploma (disregard if submitted already)
c. Transfer Credential Certificate / Honorable Dismissal Certificate
d. Original NMAT Result
e. Alien Certificate of Registration (ACR) issued by the Bureau of Immigration
f. Miscellaneous Foreign Fee (For Resident Aliens, Non Resident Filipinos and Graduate of a
Foreign College level) - $10,000.00 – NON-REFUNDABLE

NOTE:
THE SCHOOL HAS NOT AUTHORIZED ANY PERSON/AGENCY OR ORGANIZATION TO WORK ON BEHALF OF
ANY APPLICANT FOR ADMISSION.

THE ADMISSIONS COMMITTEE WILL NOT PROCESS ANY APPLICATION SUBMITTED OR FOLLOWED UP BY
THIRD PERSON.

ALL INQUIRIES, REQUESTS AND DOCUMENTS MUST BE ADDRESSED DIRECTLY TO THE OFFICE OF THE
ADMISSIONS.

ALL DOCUMENTS FILED IN SUPPORT OF THE APPLICATION BECOMES THE PROPERTY OF THE
