PARENTAL CONSENT

TO WHOM IT MAY CONCERN:

consent to the participation of my/our son/daughter in the “Research Festival” to be held at
Saint Columban College (SCC), Youth Development Center (YDC), Pagadian City on March
5, 2018.

We/I may have considered the benefits that my/our son/daughter will derive from
his/her participation in this event with the understanding that due care and precaution will be
observed to ensure his/her safety and that school officials are hereby waved with any
accountability for any untoward incident in the duration of the said activity which will be found
to be beyond their control, unless proven otherwise.
