Change Matrix Form

Once you become a doctoral candidate and begin completing the milestone documents (i.e., concept paper, or
CP; dissertation proposal, or DP; and the dissertation), a change matrix (CM) will be required to record how and
where professor-requested improvements were addressed. It is also an opportunity to ask questions for greater
clarity concerning professor comments. This document contains an ongoing record of the changes made to the
milestone documents for current and future understanding as well as to facilitate the improvement process.

Based on professor feedback on assignment 6, please complete and submit only the change matrix form below
for assignment 7.
