That M E M B E R - CO N S U M E R hereby agrees to pay Twelve Percent (12%) surcharge of the total amount
due to ALECO; The Summary of Arrearages and Consumer Subsidiary Ledger is hereto attached and shall form part of
the agreement;

That the Parties, hereto, agree that the balance of power bills and surcharges for the billing months
________________________ amounting to __________________________________________ only
(Php _____________ ) shall be paid ______________________; Provided, however, that ALECO/APEC shall
implement automatic disconnection of electric service connection for any default in the payment of the bill arrearages
