Southern Insurance Company

25 Main Street, Suite R
Milwaukee, WI 53211

Dear Sir or Madam:

Re: Mr. Mike Edmondson

I understand from Mr. Edmondson that the insurance company feels that the charges for my services on
June 30, 2017, are excessive.

Mr. Edmondson was seen on an early Sunday morning with a stab wound in his chest, which had
penetrated his lung producing an air leak into his chest wall. In addition, he had a laceration of his lung.

After consultation and review of his x-rays, his laceration was repaired. He was observed in the hospital
for two days to be sure that he did not have continuing hemorrhage or collapse of his lung.

I feel that the bill given to Mr. Edmondson is a fair one. We received, on July 31, 2017, a Southern
Insurance Company check for $40, and I feel that your payment of $40 is unreasonable. It is doubtful
that one could get a plumber to come out early Sunday morning to fix a leaky pipe for $40, and Mr.
Edmondson’s situation, in my opinion, was much more serious than would be encountered by a
plumber.

We will bill Mr. Edmondson for the remainder of the $200 balance on his account, but I want you to
know that we feel that your payment is insufficient. If he feels that the bill is excessive, we would be
glad to submit to arbitration through the County Medical Fee Committee. If this fails, I suggest we seek
help through the Wisconsin Insurance Commission.

Sincerely yours,

Charles A. Schloemer, MD
