SOP – Replacement of AF Special Rebate Cards

1. Design of existing Rebate Card for AF personnel has been changed due
to security reasons. New design has been prepared to make the card
look like a retail loyalty card rather than a svc / ID card.(new design att)
2. There will be no requirement to attach picture with application form in
future as the same will not be printed on card.(new app form att)
3. Rank / unit / fmn info will be retained in CSD Head Office and will not be
printed or stored on card or tfr to CSD shops
4. Printing of new Rebate Cards will start from 10 May and cards will be
dispatched to concerned CSD Shops (the shop where card-holder
performed last shopping) – This is only for replacement of existing cards
whereas the procedure for new applications will remain same i.e;
applications will be fwd by units / fmns / offices with covering letter and
cards will be sent to units / fmns / offices
5. Card-holders can collect the new card only upon deposit of old card at
the same / concerned CSD shop w.e.f 15 Jun 2018.
6. It is essential for rebate card-holders to obtain their new card by 30 Aug
2018 as the old cards will be blocked after the said date
7. Existing Card Holders only : In order to select your preferred shop for
card delivery, please visit sp.csd.gov.pk/rebatecards.aspx. Enter your
card no (e.g; A001111, M002222) and select a CSD shop. Your card will
be dispatched to this shop after printing. Later, you can also use this link
to track printing / delivery status of your card.
8. Though the new card will not display details of unit, picture etc it will
however display details when swiped for checking in case it is used by
unauthorized person
