Private Complainant, through the undersigned counsel, unto this Most

Honorable Court, respectfully alleges and manifests:

That the City Prosecutor’s Office has already issued the Authority to
Prosecute dated July 04, 2018. There is a need that the said authorization be
approved by the Honorable Court. Hence, the undersigned counsel now begs for
the approval of the authority to prosecute the above-entitled case issued by the
City Prosecutor’s Office of Laoag City.

That the undersigned counsel most humbly enters his appearance as

counsel for the accused in the above-entitled case;

That it is respectfully prayed that all notices and court processes shall be
forwarded to the undersigned at his office located at Diego Law Office, Room
825, 80-888 Realty Building, Gen. Luna Cor. Balintawak Sts., Laoag City,
Philippines.

WHEREFORE, it is respectfully prayed unto this Honorable Court that the

entry of appearance as counsel be noted and all of the notices and other court
processes be furnished at the aforementioned address.
