1) How to access Application Form for COC

*You can refer your TCS Pension Number in ‘Monthly PF Statement’. To access Monthly PF statement log on to
Ultimatix Employee Services My Documents  Provident Fund Monthly PF statement

3) Click ‘Save Data’. A confirmation message “Your online reference no. is xxxxxxx” appears when the Form is
submitted.

4) The complete COC application Form appears in a new tab.

5) Save a soft copy of duly filled COC application on your machine and take a print. Please ensure to save the PDF
file on local machine, if not saved, the functionality of retrieval of the same is not available on EPFO portal and
employee will have to fill fresh application again.
6) Employees should mention COC end date as one of the following whichever is earlier date

1) 3 years from COC start date. –

- For Eg: if your date of departure from India is 10-Jul-2016, then the end date will be 09-Jul-2019

- For Eg: If your date of departure from India is 12-Aug-2016, then the end date will be 11-Aug-2019

2) Your Visa End Date (applicable for PR and 457 visa / 457 Dependent Visa)– This will be mentioned in your
Visa Grant letter as “Must not arrive after”

3) Your Passport expiry date.

7) Employees travelling on short term deputation to Australia (less than or equal to 90 days) should mention COC end
date as one year from date of departure from India or passport expiry date whichever is earlier
