 That on or about 1330H June 11, 2018 take picture taking in selling area with
permit issued by store supervisor.
 That on or about 0900H June 15, 2018, I issued incident report to Julius Borje
(bagger) because using white shirt instead of his company uniform.
 That or about 1520H June 15, 2018, Mr. Chico Lopez (Granville) take picture
taking in selling area with permit issued by store supervisor.
 That on or about 1340H June 16, 2018, Marilou Abero (alphabet Soup Inc.) take
picture taking in selling area with permit issued by store supervisor.
 That on or about 1020H June 17, 2018, during my tour of duty I saw a customer
dringking mogu-mogu and I follow and monitor until paid in counter 4, with
transaction #261831.

 That on or about 1000H 01 May 2018 I issued an incident report to Joey

Llaneza (security aide) because he failed to submit his weekly
accomplishment report covering the period of April 23-April 29, 2018.
 That on or about 0927H 02 May 2018 one customer damaged one (1) bottle
of Eden Sandwich spread 220ml with item code 201272 worth of Php. 67.65
we monitor it until she left to counter #03 and pay the said damage. The
cashier on duty was Florence dufo transaction #262518.
 That on or about 0920H 05 May 2018 I issued an incident report to Darwin
Busnag (merchandiser) because he failed to monitor and pull-out his stock
22 punches datu puti adobo sa gata mix sup 180ml with item code HSE152
worth of Php. 38.05 each was found expired last Nov. 11, 2018 at the excess
shelve.
 That on or about 1154H 05 May 2018 I issued an incident report to Lovie
Batalon (checker) because of wrong item received wherein he received one
(1) case golden instant dats twin pack 200g/25/48 which is not carried in the
store instead of golden instant dats 200g/48 with item code ADJ008
resulting to pull-out the said item.
 That on or about 0800H 06 May 2018 I issued an incident report to Joey
Llaneza (security aide) because of absent without official leave.

(cashier) because of wrong quantity entered, wherein she entered 10 bottles of c2
Solo apple 230ml at 8:25 (8205) instead of 10 cases of the said item at 197.80
 On or about 1730H Dec. 29, 2017 I issued (IR) Incident Report to Rolex Dexter
Vidal (LP) because of Tambay mode at lane 1 white is co-employee busy doing
their task.
 On or about 1801H Jan. 01, 2018 I issued (IR) incident report to Kiervin Bernal
(Bagger) because he failed to tally on his cashier resulting to customer complaint.
 On a about 1801H Jan. 01, 2018 I issued (IR) incident report to Genvie Mae
Alcaide (Cashier) because of wrong quantity entered. Punched 2kg. of hansel
cracker 10’s/30 at 44.15 instead of 1 kg only under tr# 351531 resulting to
customer complaint.
