# Insulin –MIXTARD (M-W-F-SUN)


- 14 units on days WITH NO DIALYSIS BEFORE BREAKFAST

# Apridra
- Give 4 units for CBG greater than 200 mg/dl on non dialysis days
- DON’T GIVE ON DIALYSIS DAYS

Note:
Lyrica- temporarily removed as per ST LUKES BGC PAIN MGT CENTER

Myonal- temporarily removed as per ST LUKES BGC PAIN MGT CENTER

Diamicron- temporarily removed as per DOC JM CO

Warfarin- dosage adjusted due to high INR in PT test (doc abad advice)

Calci-Aid/Rocaltrol- resumed June 3 due to low TOTAL CALCIUM., being given intermittently

Occuvite- being given if possible, usually 1 tabs daily, max given is 2 tabs daily

Sodium Bicarbonate – ask if still need to continue
